Kansas is a recent addition to the Supporting Families Community of Practice which centers around the Charting the LifeCourse Framework.  Those of us in Kansas who are part of this have found this to be a very good person centered planning philosophy with easy to use tools.  What makes it especially effective is that the Charting the LifeCourse Framework has been, and continues to be, developed by families and self advocates.  Sheli Reynolds, Ph.D., who is the director of this Community of Practice is both a family member and has a strong background in self advocacy being the former director of People First of Missouri.

Hello Everyone;

I am working with our State Medicaid and DD Program in developing an implementation plan for Person-Centered Planning services in Idaho.  These would be specifically services for the facilitation of the planning process.  I would appreciate any information (and links) about state Medicaid/DD Agency programs, systems, or protocols you feel are following a good, truly person-centered process.  I appreciate any information about related training for providers/faciliators as well.
