Hi Tracy,
While not OFFICIAL documents (in terms of the planning process), we have been sharing the tools from the supporting families community of practice and from
Michael Smull/Support Development Associates. We encourage families to work through these tools (on their own or with a facilitator – which the Council can support) as a way to be more prepared for the official process.
I don’t know if you are at all familiar with the Supporting Families Community of Practice, but this stuff is fantastic and free. I’m going to attach to this
two power points I’ve used recently to share the concepts and tools. I think you can get a feel from those and there is MUCH more on the websites listed at the end of the presentations. Invite someone from Kansas City to come out and teach you this stuff.
It’s magic.
We love the Support Development Associates stuff too, and are using it, but you kinda have to be “certified” to train and use these tools and our capacity there
is low. But if you’d like to talk with my staff person who IS certified, I’m glad to put her in touch with you.

Subject: [Marketing Mail] DD Councils Request for information about State Medicaid/DD Programs using good Person-Centered Planning model
Hello Everyone;
I am working with our State Medicaid and DD Program in developing an implementation plan for Person-Centered Planning services in Idaho. These would be specifically services for the facilitation
of the planning process. I would appreciate any information (and links) about state Medicaid/DD Agency programs, systems, or protocols you feel are following a good, truly person-centered process. I appreciate any information about related training for providers/faciliators
as well.
Thank you, Tracy
