Hi MaryLee.  I've attached a document that combines the new federal performance measures and definitions with the additional measures in DD Suite that the User Group requested.  The additional measures are noted as non-federal with an "NF".  We are still in the process of updating our grantee application instructions.  I know that North Carolina had an excellent training document for applicants but I can't find a copy.
