Our waiver redesign was implemented on September 1. The link below is probably the best way to get info. There are a number of new services including expansion of private duty
and skilled nursing. Provider capacity is still a huge issue though. We are well behind our DOJ Settlement Agreement in terms of moving children out of nursing homes or other institutions.
The redesigned waivers provided a tiered rate based on Supports Intensity Scale (SIS) score that is meant to incentivize providers to serve more complex individuals. Too early
to predict how well that will work. There is also an RFP out now to develop community capacity in terms of small medical homes (basically small group homes for medically fragile individuals who don’t want to live on their own or can’t. Everything is really
still in a state of flux, probably for the next 6 months to a year.
We do not have a brain injury waiver and other than children who meet the DD Definition but otherwise, services for that population are severely lacking. Our DD Waivers (three
of them) used to be ID, DD, and Day Support, now Community Living, Family and Individual Support, and Building Independence, have new crisis prevention and intervention services built in as well to serve folks with mental health and/or behavioral needs who
are on the waivers.

In KY, we are looking for examples of waivers that provide the full range of services for:
(1)
Individuals who are MEDICALLY FRAGILE
(2)
Individuals who have DUAL DIAGNOSIS (DD and Mental Health or DD and Acquired Brain Injury)
Right now, many individuals are struggling because the services either (1) are not available in their community or (2) waivers do not cover them. Providers are refusing to work with ‘complicated’ individuals. There appears to be an increase
in the number of families seeking involuntary commitments because they feel they have no good options in communities.
We may also need suggestions for how you have addressed issues related to Work Force shortages; durable medical equipment; and a host of other related issues.
Any waivers or other information you could share would be greatly appreciated!
