The Arizona Developmental Disabilities Planning  Council  in collaboration with the two UCEDD’s serving Arizona, is featuring an upcoming webinar dedicated to a promising practice.  Youth with developmental disabilities aging out of foster care often have few resources to assist them in becoming successfully included in the community.  This webinar will explore the program “A Picture of Life” detailing person-centered planning and it’s use to assist youth approaching age 18 to be self-determined in utilizing options that are available for transition to adult and community life.
 
Please consider attending and sharing this information with colleagues and others that may have an interest in this important aspect of foster care.

You'll hear about the reality of caring for kids with disabilities in foster care, why creating a person-centered plan matters and learn about resources to increase transition success in the lives of some of Arizona's most vulnerable young people.
