The Mass. DD Council is collecting information about how co-employment models similar to Massachusetts’ “Agency with Choice” are administered in other states.  If you could take the time to answer the following questions, or send the name(s) and contact information of agency staff who can answer these questions, we’d greatly appreciate it.

 

1.      What is your state’s program called –are there different types offered?

2.      What % of your state utilizes this program vs. traditional services?

3.      If there is a provider component, how are providers paid including their specific rates?

4.      Can you confirm if the state pays the provider directly or does it come out of the allocation provided to the individual or family?

5.      What is the allocation provided for an individual or family?  If there is a range please give us the range and average allocation if latter is known.
