Where can I find the 2017-2021 State Plan Assurances?

On the ITACChelp.org website under Resources>Five Year State Plan>Assurances http://itacchelp.org/resources/five-year-state-plan/ or see attached.

Guidance on Assurances from the new 5-Year State Plan Template:

SECTION VI: ASSURANCES [Section [124(c)(5)(A)-(N)] Submit Assurances with the 5 Year State plan. Assurances only have to be submitted at the beginning of the 5 Year plan unless there have been substantive changes to the plan or the Council Chairperson or Designated State Agency has changed since the assurances have last been submitted.

DD Council can attach a signed, scanned copy of the Assurances and keep the original on file in the DD Council office.
