Subject: DD Councils children's assessment answer from Oklahoma
OK does not use a different needs assessment for children. OK does not use an assessment that is numerically scored. An assessment is completed at the point the child's app is worked
for entry into a waiver. This info along with a psychological, physical required by Oklahoma Health Care Authority (Medicaid agency) are completed to support a person centered team meeting where the family and key team members talk about key information needed
to develop a plan.
Here is the initial needs assessment form the DDS intake case manager uses to start the assessment process. This assessment format is completed with families are at the point DDS receives the funding needed
to process the application for services. It is submitted to OHCA with an updated psychological and medical report to establish the initial eligibility for services. As soon as the individual is approved to receive services, a case manager is assigned.
The newly formed team, which includes service providers and others selected by the individual/parent/guardian, uses this assessment, along with detailed information provided by other professionals , for development of the first person centered plan. Oklahoma
does not numerically score information to determine resource allocation. Each plan is individually developed and authorized by a Plan of Care reviewer administratively outside of the Case Management system. Thanks JG
