TN is using the CQL measures in our state QA system. We don’t require provider accreditation as NC & others do.

Hi Karen,
NC is a NCI state and the data is really important. That said, one of the strongest tools that that I am familiar with that measures quality from the standpoint of a defined personal outcome followed by specific actions or supports is the Council on Quality and Leadership (CQL) Personal Outcome Measures tool. I think Tennessee has embedded the CQL POMs as part of their measurement of quality. In NC, CQL is recognized as one of four options for provider accreditation (national accreditation is mandated for providers in NC).
Hope this helps!
Chris

I agree with Tracy: NCI is the gold standard for health, quality of life, and self-determination outcomes. Wish WI used it more widely!

Karen,
Have you considered looking at the indicators included in the National Core Indicators Project? We are not an NCI state but wish we were. If memory serves, they include some good items related to access to community, relationships, etc.

Hello
The State of Vermont asked me to tap our network expertise to collect ideas for indicators of the quality of community supports, to build into contracts with service providers.

Here's request from State:

Vermont is interested in what other States are using for measurement tools and/or outcome measures to help look at how well people with developmental disabilities are supported to be part of their communities and in developing relationships – including:
1. Quantitative measures – such as how much or how often people are involved in their communities – which may provide opportunities for more meaningful connections (other than what can be obtained via the NCI Adult Consumer Survey).
2. Qualitative measures – such as developing relationships, making social connections, spending time with people not paid to be with them.

Info on what your state is using as indicators -- or ideas based on your experience -- would be appreciated.

Thanks
K
