The NORC at the University of Chicago is seeking individuals with disabilities to participate in s survey about COVID-19
and they need more people with intellectual and developmental disabilities o take the survey and tell them about your experiences with COVID-19!
Help us ensure that the COVID-19 experiences of people with disabilities are heard! Please

The goal of the CHAMPS Survey is to learn about your feelings, experiences, and needs related to COVID-19. This information will be used by the National COVID-19
Resiliency Network (NCRN) so that we can support other people like you. Learn more about NCRN at

Things to know:

The survey should take you about 10 minutes.

The survey is for adults 18 years and older.
2.
This survey is private. Your name will not be tied to your responses.
3.
There is little to no risk to you.
4.
The survey is available in 11 languages (Chinese, English, French, Haitian Creole, Hmong, Korean, Portuguese, Somali, Spanish, Tagalog, and Vietnamese).
5.
