Family Toolkit to Prepare Parents/Caregivers for their Youth's Transition to Adult Care

Got Transition and its National Family Health Care Transition Advisory Group have developed a new toolkit for families to use to guide their youth’s transition from pediatric to adult health care. The Family Toolkit includes easy-to-use resources for youth and families to help youth assume more independence in taking care of their own health and using health services. Materials include a transition timeline, questions to ask your doctor, what turning 18 means for one’s health, and more. Got Transition staff and representatives of its National Family HCT Advisory Group will host a webinar about the Toolkit on October 27th at 1pm ET. Register HERE!

The National Alliance Receives Funding to Advance Quality Measurement of Transition

The National Alliance to Advance Adolescent Health/Got Transition has received funding from the Lucile Packard Foundation for Children’s Health to develop a new health care transition (HCT) quality measurement framework and a set of existing measures that can be used to evaluate different aspects of transitional care, working with a multi-stakeholder advisory group. Quality measurement is an important vehicle to accelerate HCT improvement and accountability and incentivize clinicians and health systems to provide HCT services. For more information about this exciting grant, please read the following press release.

New Report Offers Options of Medicaid Managed Care Contract Language to Expand the Availability of Pediatric-to-Adult Transitional Care

The National Alliance to Advance Adolescent Health/Got Transition has developed a new report, Medicaid Managed Care Contract Language to Expand the Availability of Pediatric-to-Adult Transitional Care, which offers options of Medicaid managed care contract provisions that states can use or adapt to improve the availability of HCT services for youth and families. Suggestions are included on the following topics: definitions, member services and education, provider network, covered services, care coordination, and quality and evaluation. National Alliance/Got Transition staff will be hosting a webinar on November 13th at 2pm ET, which will include examples of how Florida and Texas have included transition language in their Medicaid managed care contracts. CLICK HERE to register!

National Network Update: Network Café Notes and Resources

The National Network for Advancing Systems of Services for Children and Youth with Special Health Care Needs (CYSHCN), which includes Got Transition (The National Alliance), Catalyst Center (Boston University) and the AAP’s National Resource Center for Patient/Family-Centered Medical Home, has formed a Network Café, where Title V CYSHCN program directors meet monthly to discuss various topics related to COVID-19 response for CYSHCN and their families. So far, these discussions have focused on emergency preparedness, behavioral health and well-being of the Title V workforce, and racial equity, among others. The Network will continue to hold these cafés on a monthly basis. CLICK HERE for resources and notes from these Network Café discussions and contact xxxxxx@aap.org for more information.

SAVE THE DATE: Upcoming Virtual Conferences on Health Care Transition

Two annual conferences dedicated to health care transition will be held virtually later this month. On October 29-30, the Baylor School of Medicine and Texas Children’s Hospital will hold its 21st Annual Chronic Illness and Disability Conference: Transition from Pediatric to Adult-Based Care. The annual event, which is approved for CME credit, will include lectures, breakout sessions, and Q&A sessions addressing a myriad of HCT-related issues. CLICK HERE to register for the event. That same week on October 28, the Health Care Transition Research Consortium will virtually hold its 12th Annual HCTRC Symposium, a one-day event featuring research presentations and posters on a range of emerging transition topics. CLICK HERE to register for the event.

The recently released 2018-2019 National Survey of Children’s Health indicates an improvement in national HCT performance since 2016-2017, among both youth with and without special health care needs.
