Hello from Colorado. This may not totally align with the information you requested but we produced a guide to Supported Decision Making. Here are two versions:
Spanish and English Colorado has had a concerning decades-long practice around the restriction of rights of adults with intellectual and developmental disabilities in certain instances when the provider of residential or other services concludes the person is making choices that aren't consistent with his or her health and safety. with the implementation of the  CMS Final Settings Rule we've been participating in a stakeholder workgroup to reform and improve the practice to ensure the person can work with an independent advocate and hopefully incorporate SDM.
