SC’s DD Agency has an online “Provider Dashboard” that aggregates data from the quality management division into a star-rating and points system for individual services. Families and people with disabilities can also see scores on incident reporting, compliance enforcement, plans of correction, administration, and more at the provider level.

Hi everyone,

 

Our state DD agency in DC is revamping their “provider report card” which is supposed to provide performance data on all the service providers here (we have over 80). We’re engaging in a conversation about what kind of information would be helpful to include on that provider report card. Has anyone seen an evaluation or report card for service providers that seems to work well and help people and families choose a service provider?
