Attached is the one from 2015

Staff and I will be developing a new one this summer, the UCEDD works with us to get the survey out. We are thinking of holding “townhall” type of meetings for families and self-advocates to get more response.

We’d love to see Ky’s Joint Needs assessment. Can anyone share?

Thanks,

Hi Brian, here’s what CA used for the  16-21 State Plan. Like many of you, we’re developing the survey for the 22-26 plan. Glad to offer our old work if it help inform your new survey. I like that KY does a joint needs assessment. Best, Aaron

If you have survey questions that you like that you have used for State Plan development to assess needs and priorities at the front end of your planning, would you please share? I'm am looking for questions you ask people with developmental disabilities, family members, and their allies outsie of the Council. We are always looking to identify ways to improve. Thank you.

Brian
