TO:  DD Council staff members responsible for the PPR

We need your help in identifying areas where improvements can be made in PPR information and guidance materials.  Please take a few minutes to complete the two-part survey.

Part I is about ITACC guidance materials; please reflect on your recent reporting experience and help us identify areas where ITACC guidance materials can be strengthened.

Part II is about the ACL Reporting system user guide and instructions provided in the on-line template.  Your feedback will be shared with AIDD staff and the reporting system contractor.

The survey will take about 5 minutes to complete and will close at 5:00 p.m. ET on May 24, 2019.
