We have contracted with our UCEDD to develop a reporting platform. It is really amazing. They studied the requirements and reporting format and developed an input
systems all our contractors have access to. It develops quarterly reports and is very user friendly.

I am interested in knowing how Council's are collecting information about state plan projects and/or activities that are conducted by Council staff.
If you have information or a data collection/tracking form can you please share it with me? (xxxxxx@nacdd.org)
Thanks in advance,

Sheryl,
The Connecticut Council has been using attendance sheets, pre and post tests, and satisfaction surveys, however we have realized that these tools do not capture all the information required for the AIDD program performance measures.
This year we will fund an internship at the Department of Public Policy in the School of Business at the University of Connecticut. We will have a PhD student in their survey research program design and conduct surveys of all the individuals who have attended
Council programs over the past three years. We will also use this new resource to survey Connecticut citizens about needs, quality and capacity of supports and services for individuals who live with developmental disabilities.
Best,
Walt Glomb

Hi Sheryl. As you know we use DD Suite to collect narrative information and performance indicators, including the work of Council staff and members. We create a council activity for each 'internal' activity identified under each
state plan objective. We assign the
related staff and set up regular reporting intervals (programmatic reporting), to collect all the narrative and performance data. Since PPR reporting is done by state plan goal and/or objective, this information is aggregated by
the system, then edited and validated for the PPR. I've attached a screenshot that shows a typical setup and the list of council activities we had for plan goals 3 and 4 in 2018.
